Consent Form

    I, the undersigned, hereby grant permission and consent to The Den of Misfit Pets, BlackBoots Twin Ports, and The Main Club (collectively referred to as "the Organizers") to photograph, record, and/or capture electronic media of me during BlackBoots Twin Ports’ Gear Night ("the Event").

    I understand that these images, videos, or recordings may be used for promotional, educational, archival, and commercial purposes, including but not limited to social media, websites, printed materials, and marketing campaigns, without any expectation of compensation or further approval.


    By attending the Event, I confirm that I am 18 years of age or older. I voluntarily assume all risks associated with participation, including but not limited to personal injury, illness, property damage, and any unforeseen incidents arising from or in connection with my attendance.

    I agree to release, indemnify, and hold harmless the Organizers, their officers, directors, agents, volunteers, and representatives from any and all claims, demands, liabilities, losses, expenses, or damages (including legal fees) resulting from my participation in the Event.

    I acknowledge that I am responsible for my own actions and agree that the Organizers shall not be held liable for any misconduct, negligence, or rule violations by me or other attendees. I further understand that failure to comply with the Event's rules and standards may result in immediate removal from the Event without refund or recourse.


    I understand that I may revoke this authorization at any time by providing written notice to **The Den of Misfit Pets, BlackBoots Twin Ports, or The Main Club**. However, I acknowledge that revocation will not apply retroactively and will not affect any media already published or used prior to receipt of my written notice.


    I acknowledge that photographs, videos, and related materials will be stored securely, accessed only by authorized individuals, and retained for as long as they remain relevant. Once deemed unnecessary, they will be securely archived or deleted.
